Primary Care Advanced Practice Provider

Opening in Titusville, Florida

Health First is seeking a dedicated APRN or PA to join our growing Primary Care team in Titusville, Florida. This provider will work in close partnership with a physician to deliver comprehensive outpatient care and support the long-term health and wellness of an established patient population.

Position Highlights

•        Outpatient Primary Care setting
•        Provide care for acute and chronic conditions, preventive services, and health maintenance
•        Develop and implement patient-centered plans of care using established clinical protocols
•        Collaborative, team-based practice environment

Qualifications

•        Licensed APRN or PA
•        Previous Primary Care experience strongly preferred
•        Active Florida license required
